The Diabetes Self-Management Program is provided by Texas Health in collaboration with Dallas Area Agency on Aging. It is designed to help you better manage your diabetes, prevent complications, and improve your overall quality of life. Caregivers and family members are also welcomed to attend and participate in the program.
This program is presented in a virtual format using videoconferencing. The full program consists of 2.5-hour interactive small group sessions, once a week for six weeks. The participants of the program learn new skills, share their experiences, make weekly action plans, and help each other solve problems as they work together in creating and carrying out their self-management programs. Participants are provided a free copy of a relaxation CD and the Living a Healthy Life with Chronic Conditions book as resources to assist them on their journey to improved health and well-being.
