Pre-regisration by Tuesday 11/22 is required for this clinic. If you are interested in getting your COVID booster in Southwick on 12/1 please contact MaryAnn Deming at 413-427-5532 for registration forms. In addition, if you are needing transportation, the Southwick Senior Center has offered to send its van to pick folks up at the PSC (time TBD dependent on need).
