(Woodland, CA) – For the second time, Yolo County Health & Human Services Agency (HHSA) collaborated with CommuniCare Health Centers, Dignity Health, Sutter Health, and Winters Healthcare, with support from Community Health Insights, to develop a new three-year Yolo County Community Health Needs Assessment 2022-2024 (CHNA).

"CHNAs are powerful tools for community leaders and residents to better understand our most significant health needs," said Board Chair, Supervisor Angel Barajas. "We can now translate this information into action, creating or enhancing programs meant to help support greater access and more equitable health outcomes."

The top health needs identified in the 2022 CHNA are:

Access to Basic Needs Such as Housing, Jobs, and Food
Access to Mental/Behavioral Health and Substance Use Services
Injury and Disease Prevention and Management
Active Living and Healthy Eating
Access to Quality Primary Care Health Services
System Navigation
Access to Specialty and Extended Care
Increased Community Connections
Safe and Violence-Free Environment
Access to Functional Needs
Access to Dental Care and Preventive Services
HHSA invites community partners, health care centers, and residents to join HHSA staff as part of a collaborative workgroup that will develop a three-year Community Health Improvement Plan (CHIP). The purpose of the CHIP is to create an action plan in response to the top significant health needs identified in the CHNA.

This process will begin in early Fall 2022 and will coincide with the release of the Yolo County Community Health Assessment (CHA). The CHA features the CHNA data and additional data sources that will help identify geographic areas or communities disproportionally impacted by health disparities to be addressed in the development of the CHIP.

About the Community Health Needs Assessment: The information contained in the CHNA will help Dignity Health Woodland Memorial Hospital, Sutter Davis Hospital, and other healthcare organizations guide local planning and decision-making for community health investments.
