Chinatown Public Health Center (CPHC) is one of twelve San Francisco Department of Public Health Community-Oriented Primary Care health centers. CPHC has been serving San Franciscans and the Chinese community since 1929. Since breaking ground in 1969 on the current and fifth site in Chinatown, CPHC has transformed itself from a traditional public health center into a full-service, community-oriented, patient-centered medical home that is part of San Francisco's safety net delivery system. We offer a full array of services including primary care for adults and children, prenatal care, team-based care, population health disease registries, integrated behavioral health, pediatric dentistry, community health education, public health nutrition, and Women, Infant,, and Children Nutrition Supplemental Services. CPHC serves close to 6,000 active patients, many of whom are socioeconomically disadvantaged and/or culturally isolated. We serve our clients with cultural sensitivity and pride.
