Carolyn Kubiak, D.O., is a board-certified family medicine physician at Baptist Health. Before joining Baptist Health in 1995, Dr. Kubiak completed her family practice residency at the University of Miami, Jackson Memorial Hospital Medical Center Program. She earned her D.O. from Southeastern University of the Health Sciences of Osteopathic Medicine.
